A blind validation is presented of a novel saliva-based oestradiol and progesterone assay carried out in samples collected in independent IVF clinics. Obtained results of this randomized double-blind sham-controlled clinical trial has shown that penile electromagnetic shockwave therapy may improve erectile function, to a modest extent, on certain patients that do not respond to PDE5I; making it an alternative for vascular ED patients that reject more invasive therapies.

Our study found that the use of low dose vaginal misoprostol prior to office hysteroscopy is associated with lower reported pain and tenaculum utilisation during the procedure. However, vaginal misoprostol prior to successive office hysteroscopy and endometrial biopsy failed to decrease the reported pain, and the overall pain score was higher than hysteroscopy alone.

Of those patients who experienced vasectomy reversal failure 50% elected to use cryopreserved sperm that had been procured at the time of initial reversal. Given the potential for early or late failure, cryopreservation of sperm at the time of vasectomy reversal should be routinely offered as a means of avoiding the added expense and potential morbidity of future surgical sperm retrieval.

Analysis of 92,471 insemination cycles from 37,553 patients were included in this study found that IUI pregnancy is optimized with TMSC of 9 million, below which the rates gradually decline. Although rare, pregnancies were achieved with TMSC of<0.25 million.

Frozen embryo transfer cycles in GCs with DET were associated with more preterm births and lower birth weights compared with those of SET. Intended parents and GCs should be counseled that DET is associated with greater risks of adverse pregnancy and perinatal outcomes, which mitigates higher live birth rates. The use of PGT-A did not appear to improve the live birth rate. 583 FET cycles were analyzed.

Authors of this study concluded that it is reasonable to attempt IVF-ICSI with partner sperm in patients with genetic causes of male infertility. Patients should be counseled regarding the possibility of no sperm being available from the male partner, poor/failed fertilization, and genetic implications for potential offspring. Contingency plans, including IVF with donor sperm backup or oocyte cryopreservation, need to be made for these scenarios.

An abnormal number of chromosomes in an embryo (either too many, or too few) will likely cause failed implantation or miscarriage. As a female enters her mid to late 30’s, the quality of her eggs begins to significantly decline, making conception and a resulting live birth, more difficult to achieve. For this reason, many women in this age group, using their own eggs, utilize PGT-A testing to identify chromosomally normal embryos for transfer, increasing their chances of success. In this study researchers found, PGT-A testing of embryos resulting from an egg donor, a women in her twenties with presumably higher quality eggs, did not improve the likelihood of live birth for recipients. This outcome suggests in many situations, embryo testing, with its additional time required and expense is not beneficial when using the egg of a donor.

The findings of this study demonstrate that the presence of Endometriosis alone does not impact the pregnancy or live birth rates in frozen embryo transfers nor does it impact the percentage of embryos that are chromosomally normal. This study looked at frozen embryo transfers using chromosomally normal (euploid) blastocyst stage embryos. The observation is significant for understanding the proper management of infertile women with endometriosis.

Natural cycle IUI should be considered as a first line treatment for women who ovulate regularly and plan to use donor sperm inseminations. This study shows ovarian stimulation in donor sperm insemination cycles, resulted in a < 4% increase in clinical pregnancy rates and < 1% increase in ongoing pregnancy. While the rates of ongoing pregnancy are similar to that of stimulated cycles, the chance of an ongoing multiple gestation pregnancy (twins or triplets) is four times higher.

The intention of publicly reporting treatment outcomes (SART or CDC), is to drive improvement to fertility treatment, promote trust between patients and their medical providers, and inform consumers before selecting their fertility center of choice. This study found that public reporting of treatment outcomes, also resulted in providers modifying their practice, limited participation of trainees in procedures, and the perception that other physicians denied care to poor-prognosis patients in an attempt to have the best possible reported treatment outcomes.

This study found, the detection rate of adenomyosis, a condition in which the inner lining of the uterus breaks through the muscle wall of the uterus, was significantly higher when ultrasound was performed by an expert gynecologic sonologists compared with radiologists. The presence of uterine fibroids, benign tumors of the uterus, significantly decreased detection rates of adenomyosis regardless of specialty.
